  1. Strona internetowa www.centrummente.pl (dalej: „Strona Internetowa”) jest prowadzona przez Valor sp. z o.o. Rynek Główny 39/5 31-013 Kraków, NIP 6762634203 -, e-mail: witaj@centrummente.pl (dalej: „Administrator”).

  2. W ramach działalności Strony Internetowej mogą być zbierane, przetwarzane i wykorzystywane dane użytkowników Strony Internetowej (dalej: „Użytkownicy”), w tym dane osobowe.

  3. User data may be collected through the use of cookies.

  4. In addition, information may be collected about the user's IP address, the time of arrival of the request and sending of the response, the address of the website from which the User was redirected to the Website and the type of software the User uses. This information is used for the purposes of administering the Website and creating statistics and analysis.

§ 2

  1. Administratorem danych osobowych Użytkowników jest Valor sp. z o.o., e-mail: witaj@centrummente.pl (dalej: „Administrator”).

  2. The Administrator through the Website may collect and then process personal data for purposes such as:

    1. Making an appointment for psychology and psychotherapy and implementing services;

    2. conducting a webinar;

    3. respond to the User's inquiries or correspondence addressed to the Administrator;

    4. conducting by the Administrator direct marketing of its own products or services (newsletter);

    5. Statistics and analysis of User behavior on websites.

  3. The Administrator processes personal data on the following grounds:

    1. In the case of scheduling appointments for Users and performing services - based on the performance of contracts concluded with Users (Article 6(1)(b) of the DPA);

    2. in the case of conducting a webinar - based on the performance of contracts concluded with Users (Article 6(1)(b) RODO);

    3. in the case of data processed for the purpose of responding to User inquiries or correspondence - on the basis of the Administrator's legitimate interest in responding to User inquiries (Article 6(1)(f) RODO);

    4. in the case of data processed for the purpose of direct marketing of the Administrator's own products or services - on the basis of the Administrator's legitimate interest, which is to promote the Administrator's business (Article 6(1)(f) RODO);

    5. in the case of data processed for the purpose of statistics and analysis - on the basis of the Administrator's legitimate interest, which is to verify the Users' use of the Website (Article 6(1)(f) RODO).

  4. The Administrator may share Users' data with its subcontractors (entities it uses for processing) such as service providers and IT solutions.

  5. The Administrator may use IT service providers that are based outside the European Economic Area. As such, data may be transferred outside the EEA. The Administrator shall only use IT services provided by entities established in countries that have been found by the European Commission to provide an adequate level of protection, from entities that process personal data on the basis of the Standard Contractual Clauses adopted by the European Commission, referred to in Article 46 of the RODO, concluded between the Administrator and that entity, or that process data based on the principles of the Data Privacy Framework.

  6. In the case of data processed for:

    1. Implementation of contracts concluded with Users - data will be kept for the period necessary to assert one's own claims or defend against claims made against the Administrator in connection with the performance of the contract;

    2. responses to inquiries or correspondence from Users - for a period of 3 years from the moment of sending the inquiry or correspondence;

    3. conduct by the Administrator direct marketing of its own products or services - the data is processed until the User objects or until the consent to receive commercial information is withdrawn; 

    4. conduct statistics and analysis of Users' behavior on the Website - data are processed for a period of 3 years from the date of collection.

  7. The user to whom the data relates has the following rights:

    1. The right to access personal data and the right to obtain a copy of it - The User may request information from the Administrator as to whether his or her data is being processed, and if confirmed, the User may request access from the Administrator. The User may also request from the Administrator a copy of the processed personal data. Providing further copies of the data may require the User to pay a fee related to administrative costs;

    2. The right to request rectification of personal data - the User may request the Administrator to rectify his/her incorrect personal data or complete incomplete personal data, taking into account the purposes of processing;

    3. Right to request erasure of personal data - You may request the Administrator to erase personal data concerning you if one or more of the following cases apply:

      1. personal data are no longer necessary for the purposes for which they were collected or otherwise processed;

      2. Personal data is processed in violation of the law;

      3. personal data were processed illegally;

      4. User's consent, which is the basis for data processing in accordance with Article 6(1)(a) of the RODO. 6(1)(a) RODO, while there is no other legal basis for processing personal data;

      5. The User objects to the processing of personal data on the basis of Article 21(1) of the RODO, and at the same time there are no valid legitimate grounds for the processing overriding the interests, rights and freedoms of the data subject, or grounds for establishing, asserting or defending claims; or if the User objects to the processing of personal data on the basis of Article 21(2) of the RODO;

      6. an obligation to delete personal data resulting from the applicable European Union legislation has been imposed on the Administrator;

    4. Right to request restriction of processing of personal data - You may request that the Administrator restrict the processing of personal data concerning you when one or more of the following occur:

      1. The user questions the accuracy of personal data - for a period that allows the Administrator to verify the accuracy of such data;

      2. processing is unlawful, and the User objects to the deletion of personal data, requesting instead a restriction on its use;

      3. The Administrator no longer needs the personal data for the purposes of processing, but they are required by you to establish, assert or defend claims;

      4. The user has objected to the processing in accordance with Article 21(1) of the RODO until it is determined whether the legitimate grounds on the part of the controller override the grounds of the data subject's objection;

    5. The right to transfer personal data;

    6. The right to object to the processing of personal data;

    7. The right to lodge a complaint to the supervisory authority - in Poland this is the President of the Office for Personal Data Protection.

  8. In order to exercise the above rights, the User should contact the Administrator.

  9. Provision of data is voluntary, but without it it will not be possible to provide services by the Administrator, participate in the webinar, receive commercial information or contact the Administrator.

  10. The Administrator does not make a decision that is based solely on automated processing, including profiling.

§ 3

  1. As part of the activities of the Website, cookies stored in the Users' terminal devices are used. The use of cookies should be understood as their storage and access to them by the Administrator.

  2. Cookies are IT data, in particular text files, which are stored on the User's terminal device and are intended for use on websites. Cookies usually contain the name of the website from which they originate, the time they are stored on the end device, content (e.g. action identifiers) and a unique number.

  3. Due to the length of time for which cookies are installed on the User's terminal device, the cookies used on the Website are divided into two main types:

    1. session cookies (session storage) - are temporary files that are stored on the User's terminal equipment until the session expires (e.g. leaving the Website, deletion by the User, or turning off the software);

    2. Preference - allows the Website to remember information that changes the way the Website works or looks, such as your preferred language or the region where you are located.

    3. Statistical - helps to understand how Users interact with the Websites by collecting and recording information anonymously;

    4. Marketing - they are used to monitor visitors to the Website. Their purpose is to display advertisements that are relevant and interesting to Users;

    5. unclassified - files that are difficult to assign to one of the specific categories.
